چکیده

Introduction Purkinje fibers serve as triggers and perpetuators of ventricular fibrillation (VF). Although catheter ablation can be an effective therapy for Purkinje-related premature ventricular contractions (PVCs) and triggered VF, it is essential to detect the exact site of the origin of the PVCs by mapping Purkinje potentials (PPs) precisely. Here, we report a case of Purkinje-related PVCs/VF successfully treated with catheter ablation guided by mapping using a PentaRay catheter.

Successful catheter ablation of electrical storm after myocardial infarction.

BACKGROUND We report on 4 patients (aged 57 to 77 years; 3 men) who developed drug-refractory, repetitive ventricular tachyarrhythmias after acute myocardial infarction (MI). All episodes of ventricular arrhythmias were triggered by monomorphic ventricular premature beats (VPBs) with a right bundle-branch block morphology (RBBB). METHODS AND RESULTS Left ventricular (LV) mapping was performed...

Radiofrequency catheter ablation for treatment of premature ventricular contractions triggering ventricular fibrillation from the right ventricular outflow tract in a patient with early repolarization syndrome

Introduction Ventricular fibrillation (VF) triggered by premature ventricular contraction (PVC) most frequently originates from the His-Purkinje system and right ventricular outflow tract (RVOT) in patients with idiopathic VF. Radiofrequency catheter ablation (RFCA) of triggering PVCs is reportedly useful for the successful elimination of VF. The diagnosis of early repolarization (ER) syndrome ...

Trigger elimination of polymorphic ventricular tachycardia and ventricular fibrillation by catheter ablation: trigger and substrate modification

Ventricular fibrillation (VF) is a malignant arrhythmia, usually initiated by a ventricular premature contraction (VPC) during the vulnerable period of cardiac repolarization. Ablation therapy for VF has been described and increasingly reported. Targets for VF triggers are VPCs preceded by Purkinje potentials or from the right ventricular outflow tract (RVOT) in structurally normal hearts, and ...

Purkinje-related arrhythmias part ii: polymorphic ventricular tachycardia and ventricular fibrillation.

There has been growing evidence that the Purkinje network plays a pivotal role in both the initiation and perpetuation of ventricular fibrillation (VF). A triggering ventricular premature beat (VPB) with a short-coupling interval could arise from either the right or left Purkinje system in patients with polymorphic ventricular tachycardia (VT) or VF, and that can be suppressed by the catheter a...

Conversion to Purkinje-Related Monomorphic Ventricular Tachycardia After Ablation of Ventricular Fibrillation in Ischemic Heart Disease.

BACKGROUND Catheter ablation is an effective therapy for ventricular fibrillation (VF) arising from the Purkinje system in ischemic heart disease. However, some patients experience newly emergent monomorphic ventricular tachycardia (VT) after the ablation of VF. We evaluated the prevalence and mechanism of monomorphic VT after VF ablation. METHODS AND RESULTS Twenty-one consecutive patients w...
